justcarlson traced the stale state to Quickshell 0.3.2 unlock signaling and supplied a patch that explicitly refreshes the mirrored lock state. They reported successful repeated locking and four suspend tests with the patch. Lifferado independently reported that locally applying the Service.qml change restored subsequent locks and idle locking on a ThinkPad.
